This patient had a severe anterior and posterior open-bite. The only alternative to surgical treatment was orthodontic treatment with 4 extractions.
But in this case, the lower right first molar was badly fractured and her prognosis was poor. It was therefore decided to extract this tooth (much larger than a premolar) and to extract second premolars in the other three quadrants to correct crowding, biprotrusion (protruding teeth), open-bite and left posterior cross-bite.
With an asymmetric technique, all these problems were solved without the need of surgery.
